Can you explain a bit more what you wanting to know - your question doesn't make sense?

Is it "does an installed update uninstall if the WSUS content is deleted" ?
If so, the answer is NO - installed updates will not uninstall unless specifically approved for uninstall (and capable of being uninstalled)

Re: How can i clean the WSUS database?

WSUS will attempt to re-download it.
You will also need to go into options and select the "do not store updates locally - download direct from Microsoft" one, then you can delete the WSUS content safely.

Alternatively you can move WSUS to a different drive (one of the SBS options) if that would help - storing updates locally is generally a good thing.

Have you run the WSUS cleanup, possibly after removing un-needed categories and languages?

          I run the WSUS cleanup

          I'll do as you said
First select the "do not store updates locally - download direct from Microsoft
          Then i'll delete the files

          Thank you very much
