Now the email is sent to the correct email address and usually after several minutes if the sender attempts to send the email again, it goes through just fine. At the time that this occurs, there are no errors in the event viewer and no logs in the exchange server.
I also searched the firewall and i cant see any logs for that email message...
From all that I read on this error, everyone reports that it means the mailbox doesn't exist. Has anybody every encountered an issue like this?

First thing I would do is check your DNS records. Ensure that the MX record is correct and all A records being used by the MX record are pointing to the correct IP address.

      Thanks for the reply,
The DNS and MX records are fine.
After doing some testing turns out it's the FW that marks these random emails as bulk and that's the reply bounce-back that's sent out.

Re: Bounce-back error “550 Requested action not taken: mailbox unavailable”

So After eliminating the issue with the firewall, i still have the same random bounce backs with the same 550 Requested action not taken: mailbox unavailable error.
The firewall shows no trace of these emails even getting to it, and neither does the exchange server.
        This is getting really frustrating.

Re: Bounce-back error “550 Requested action not taken: mailbox unavailable”

If there is no trace in the firewall of a connection attempt, then the remote site isn't even looking at your server. You have to look at the DNS records again.

I would use some of the external tools to verify the information is correct around the world. https://www.whatsmydns.net/
