Connect your Exchange server to your router?

Alternatively give us more information that would help us solve the problem. A good place to start is to go to www.testexchangeconnectivity.com and run the tests, then report back with the results.

If you are on a dynamic iP address then most locations will not accept email being delivered directly from your server. You will need to send your email out through a smart host - probably your ISP's SMTP server.

Inbound email, you will need to first discover if your ISP blocks inbound port 25. If it does not then you will need to use a dynamic DNS service to keep the IP address updated, then change the MX record to use the Dynamic DNS host name.
